To better serve you, we have provided a printable Practice and Associate Form that you should fill out and fax or mail to us. The fax number and mailing address are contained on the form.

In addition to faxing or mailing your listings directly to our Student Affairs office, you may also want to advertise on our Chiropractic Portal. This is a free service. You will need to register to be able to post and modify your advertisements. Each registered user is responsible for maintaining his/her own portal advertisement.
